Columbine Poudre Home Care provides home care and health education. We pride ourselves in providing high quality,efficient, convenient and cost-effective health care in the homes of our patients. Home care requires a team effort involving professionals, paraprofessionals, patients and their families.
When a family finds that a loved one can no longer manage without assistance, Columbine Poudre Home Care is there to provide vital services. Home care can be assessed for a variety of medical needs including IV treatment, wound care, post surgical care, and assistance needed during rehabilition following an accident or illness. All services provided follow a written plan of treatment established and reviewed by a physician, and access to an on-call Registered Nurse 24 hours a day.
